CLINICAL CASE PRESENTATION ON LIMB-GIRDLE MUSCULAR DYSTROPHY

Highlights of the case:

Case discussion was done on the 13 year old male patient with no history of HTN, T2DM now presenting with complaints of Difficulty in getting up from sitting position and to walk since 1 year.Patient also reports difficulty when rising from a squatting position(since he was 3 years old) and climbing chairs and stairs since 1 year. For the above-mentioned complaints, the patient was taken to a nearby allopathic hospital where relevant investigations were conducted, including CBC, serum chloride, MRI of the thoracolumbar spine, MRI of the brain, and CPK. All reports returned normal except for elevated CPK levels.In ayurveda diagnosis was done as anukta vata vyadhi.Treatment plan was proposed accordingly. Sarvanga abhyanga with Ashwagandhabala lakshadi taila followed by Shashtikashalipinda sweda followed by Matrabasti with Ashwagandha ghrita(40 ml)-05/05/2024-19/05/2024 and advised oral medication like brihatvata chintamani,balarishta ,ajamamsa rasayana,neurocare drops 10 drops with milk.Discussion was done on probable ayurvedic diagnosis and intervention.

 Kantaraju, presenting with reduced strength and an inability to walk without support for a year, also experienced heaviness and tingling in his lower limbs. Examination revealed decreased crude touch, two-point discrimination, and graphaesthesia, with exaggerated deep tendon reflexes. MRI identified a peripherally enhancing intradural extramedullary lesion at D8-D9, likely a calcified meningioma or sequestrated disc. He was treated with Dashamoola kashaya parisheka, Prushta basti with murivenna taila, and Matra basti with ashwagandha yamaka and sahacharadi 7 avarti, resulting in his ability to walk without support.

With the advances in biotechnology, the recent discoveries of gene therapy and genetic modelling have helped in the advanced study of genetic disorders. With the knowledge of genetic causation, a specific therapy targeting the gene/chromosome can be understood. Some of the successful researches have been conducted in genetic modelling in diseases such as haemophilia, sickle cell anemia and spinal muscular dystrophy. Brief explanation was given in Ayurgenomics. The seminar was concluded with active discussion on various references of bheeja, bheejabhaga, beehabhaga avayava, kulaja vyadhi in ayurveda and its correlation with Genetics.








SEMINAR PRESENTATION ON -PATHYA APATHYA VIVECHANA IN HRIDROGA

 According to ayurvedic texts the Utpatti of Hridaya is from the essence of rakta and kapha ,predominantly from matruja bhava which  develops into a  muscular organ.The term “Hridaya” was first seen in Atharva Veda and was considered as an organ system comprising sirastha hridaya i.e. brain and urustha hridaya i.e. heart. 

This urustha hridaya in Samhitas was considered to be hollow organ (kostha), made up of two pesi, and three mandala sandhis, and in a shape of lotus and connected with ten maha dhamanies i.e. pulsating vessels which provide  blood, nutrition, oxygen 

 Thus immunity to the entire body  anatomically, physiologically and functionally urastha hridaya is similar to that of heart. Cardiovascular diseases refers to the following four entities,

CORONARY ARTERY DISEASE/CORONARY HEART DISEASE

CEREBROVASCULAR DISEASE

PERIPHERAL ARETRY DISEASE

AORTIC ATHEROSCLEROSIS

PATHYA AHARA-Puraanraktshali,  Jaangal pashu pakshi  mamsarasa,  Mudga ,kulatha yusha, khandayusha ,aamra, dadima, shunti,   kadali,  karavellaka, purana kushmanda,eranda taila,draksha,takra ,patola,

Mamsa varga(Meat): Jangala mamsa(meat of animals of desert),Ena mamsa(back deer),Krakara mamsa(flesh of snipe) shulya mamsa (meat pierced with an iron rod and roasted on burning coal/ flame)prepared with ghee  mamsa rasa(meat soup)

Gorasavarga (Milk and Milk products) : navaneetam (butter) ksheeram(milk), takram(butter milk)

 Madhu varga (honey): Madhu(honey).

Harita varga (leafy vegetables): shigru(moringa oleifera) Tikta saka(vegetables of bitter taste),upodika (palak)

Kritanna kalpana(prepared foods ) Odana(porridge), Mudga soup(green gram soup),Laja manda(scum of gruel prepared with fried paddy),Dadimaamalaki yusha(soup of pomogranate and indian goosberry),Khada and kambalika (prepared from cooked pulses or vegetables,added with butter milk and processed with spices and salt),Raga and shadava( thick sauce-as mixture of sugar, bida lavana,saindhava lavana,vrikshamla, juice of parushaka, jambuphala and rajika used as a syrup , guda samyukta dadhi(curds added with jaggery),.

PATHYA  VIHARA-Svedana, Vamana, Basti, Virechana, Vishraam, Langhan ,Hridya (cardiac tonics/cardio-protectives in Ayurveda)-


APATHYA AHARA-Aavi dugda,madhuka, virudda anna,guru paki ahara,Purana patra shaka,Naveena anna, ksheera ikshu vikrati,Consumption of water after food,pishta anna,abhishyandi ahara

APATHYA VIHARA-Excessive vyayama,Atiyoga basti,virechana,vamana,veghdharana ,chinta,krodha,bhaya, veghadharana 

 To protect from any type of vitiation or injury a wise person should take much vigilance by observing proper pathya and apathya and by Judicial use of Hridya (cardiotonics or cardio – protectives ) as a prevention of cardiovascular diseases

Case discussion was done on 50 year old male patient presenting with c/o increased frequency of loose stools with mucous 4-5 times/day which is alternating with constipation occassionally since 7years .In Ayurveda, diagnosis was done as Vataja grahani, treatment was planned accordingly.Snehapana with pipalyadi ghrita,Sarvanga abhyanga with karpooradi taila and nirgundi taila,virechana with Nimbamrita eranda taila and advised oral medications like Hingwashtaka churna and chitrakadi vati. Discussion was done on probable ayurvedic diagnosis and intervention
