Ayurvedic Management of Vata-Kaphaja Gridhrasi: A Clinical Case Insight

A 35-year-old female, working as a computer operator, approached our hospital with complaints of chronic low back pain radiating to both lower limbs for the past 1.5 years, along with numbness and heaviness in the legs for one year. Her occupation involved prolonged sitting and standing, with a lifestyle marked by excessive walking and disturbed sleep. MRI revealed a posterior disc bulge at L3/L4 and L4/L5 compressing the traversing nerve roots, indicative of early lumbar spondylosis.

Based on classical Ayurvedic evaluation, she was diagnosed with Vata-Kaphaja Gridhrasi, a condition characterized by radiating pain along the path of the sciatic nerve. The treatment was planned as per the principles of Charaka Samhita, focusing on Srotoshodhana, Vatashamana, and Snayugata Roga Chikitsa.

Initial management involved Koshtha Shodhana with Eranda Taila, followed by Snigdha Agnikarma using Sahacharadi Taila and Eranda Patra Pottali. Siravyadha (bloodletting) was performed at the gulpha region, resulting in a marked reduction in heaviness and partial pain relief. This was followed by Kati Basti, Sarvanga Abhyanga, and Patrapinda Sweda using Sahacharadi and Mahanarayana Taila. Yoga Basti was administered in a classical pattern with customized Anuvasana and Niruha formulations. For internal medication, Shankha Vati and Shephalika Patra Kwatha were prescribed.

On follow-up, the patient reported significant relief in symptoms. A second course of Panchakarma was administered with modifications, including the use of Ksheerabala Taila and GTG Taila in Basti. This case highlights the holistic and sustained efficacy of Ayurvedic interventions—particularly Siravyadha, Agnikarma, and Basti—in the management of chronic Gridhrasi.
