GRIDRASI WITH KATIGRAHA W.R.T LUMBAR SPINE STENOSIS

A 50-year-old male farmer, Neelappa, presented with progressive low back pain radiating to both lower limbs, numbness, tingling, stiffness, and walking difficulty for six months. MRI confirmed multi-level cervical and lumbar spinal canal stenosis with nerve root compression. Ayurveda diagnosed it as Gridhrasi with Katigraha, involving Vata-Kapha imbalance and degeneration of Mamsa, Asthi, and Snayu.

He received Ayurvedic management including Sarvanga Abhyanga, Pinda Sweda, Ksheera Vaitarana Kala Basti, and herbal formulations like Rasnasaptaka Kashaya and Trayodashanga Guggulu. Supportive physiotherapy and yoga (Bhujangasana, Shalabhasana) were advised. Functional scores improved — RMQ from 22 to 14, showing 36% improvement.

The discussion highlights Margavarana and Dhatukshaya as key factors, with Basti therapy pacifying Vata and nourishing tissues. This case shows integrative Ayurvedic treatment can significantly relieve symptoms and improve function, potentially avoiding surgery in moderate spinal canal stenosis.

This presentation on Raktavaha Srotovikara offers an in-depth Ayurvedic view on the structure, function, and pathological conditions of blood and its transport channels. Rakta is described as Jeevanam Dhatu, vital for life, complexion, and mental well-being, formed in the liver and spleen under the influence of Ranjaka Pitta. The vitiation of Rakta and Pitta dosha due to faulty diet, lifestyle, and mental factors like anger leads to diseases such as Raktapitta, Kamala, Vatarakta, Kustha, Shvitra, and Visarpa. The treatment protocol includes Nidana Parivarjana, Shodhana therapies like Vamana, Virechana, Raktamokshana, and Basti, along with Shamana chikitsa using herbs such as Sariva, Manjishtha, Chandana, and Rasayana therapy for long-term healing and prevention. Disease-specific management with diet, lifestyle, external therapies, and internal medications is emphasized based on dosha-dushya involvement and roga-avastha, highlighting a holistic and patient-specific approach.

A 59-year-old male patient, a hawker by profession, presented with chronic low back pain and stiffness—worsening over the past week—along with disturbed sleep and eye discomfort. He was previously treated multiple times with Ayurvedic therapies for Ankylosing Spondylitis (AS) and had a history of absolute glaucoma in the right eye leading to vision loss.

On admission, clinical evaluation showed loss of lumbar lordosis, reduced spinal movements, and a BASFI score of 7.6 indicating severe functional limitation. Diagnosis confirmed Ankylosing Spondylitis (Katigraha in Ayurveda), with vata dosha, asthi, and sandhi involvement, and asthivaha srotodushti. Treatment included abhyanga, swedana, tikta ksheera basti, agnikarma, and internal medications like Trayodashanga guggulu and Guggulu tiktaka ghrita.

After 9 days of therapy, the patient reported about 60% pain relief and 70% improvement in mobility and daily activities. Post-treatment BASFI score reduced to 4.8, indicating moderate functional limitation, with better lumbar flexibility and improved Schober’s test score, suggesting significant therapeutic progress through Ayurvedic management.

Ayurvedic Management of Vata-Kaphaja Gridhrasi: A Clinical Case Insight

A 35-year-old female, working as a computer operator, approached our hospital with complaints of chronic low back pain radiating to both lower limbs for the past 1.5 years, along with numbness and heaviness in the legs for one year. Her occupation involved prolonged sitting and standing, with a lifestyle marked by excessive walking and disturbed sleep. MRI revealed a posterior disc bulge at L3/L4 and L4/L5 compressing the traversing nerve roots, indicative of early lumbar spondylosis.

Based on classical Ayurvedic evaluation, she was diagnosed with Vata-Kaphaja Gridhrasi, a condition characterized by radiating pain along the path of the sciatic nerve. The treatment was planned as per the principles of Charaka Samhita, focusing on Srotoshodhana, Vatashamana, and Snayugata Roga Chikitsa.

Initial management involved Koshtha Shodhana with Eranda Taila, followed by Snigdha Agnikarma using Sahacharadi Taila and Eranda Patra Pottali. Siravyadha (bloodletting) was performed at the gulpha region, resulting in a marked reduction in heaviness and partial pain relief. This was followed by Kati Basti, Sarvanga Abhyanga, and Patrapinda Sweda using Sahacharadi and Mahanarayana Taila. Yoga Basti was administered in a classical pattern with customized Anuvasana and Niruha formulations. For internal medication, Shankha Vati and Shephalika Patra Kwatha were prescribed.

On follow-up, the patient reported significant relief in symptoms. A second course of Panchakarma was administered with modifications, including the use of Ksheerabala Taila and GTG Taila in Basti. This case highlights the holistic and sustained efficacy of Ayurvedic interventions—particularly Siravyadha, Agnikarma, and Basti—in the management of chronic Gridhrasi.

The presentation discusses Rasavaha Srotovikaras, conditions arising from the vitiation of Rasavaha Srotas, whose roots lie in the hridaya and dasha dhamanis. Improper diet and lifestyle disturb doshas and impair rasa dhatu, leading to Rasapradoshaja Vikaras such as jwara, pandu, aruchi, and klaibya. Langhana (lightening therapy) is emphasized as the first line of treatment, especially in ama-dominant conditions, followed by swedana, tikta rasa dravyas, and dietary modifications like yavagu. Depending on the stage and type of disease, treatments such as shodhana (vamana, virechana, basti), snehapana, and shamana are adopted, with specific protocols detailed for each condition like jwara, madatyaya, and kshayaja klaibya.
