VATAJA GRAHANI [IRRITABLE BOWEL SYNDROME]

A 19-year-old female patient, Ramya, presented with symptoms of altered bowel habits, urgency for defecation immediately after meals, and unsatisfactory evacuation, suggestive of Grahani (~Irritable Bowel Syndrome).

Treatment:Patient was administered Ksheerabasti in a Yoga Basti pattern (alternate Niruha and Anuvasana with milk-based medicated enema), targeting Agni Deepana, Srotoshodhana, and Vata-Pitta shamana.

Outcome:Post-treatment, the patient showed marked clinical improvement, with reduced bowel frequency, better evacuation, and decreased urgency. The progress was objectively assessed using the IBS Severity Scoring System (IBS-SSS), demonstrating significant score reduction, indicating effective management of Grahani through Ayurvedic intervention.

TREATMENT PROTOCOL OF MUTRAVAHA AND PURISHAVAHA  SROTO VIKARA

The presentation provides a detailed Ayurvedic approach to the diagnosis and treatment of Mutravaha and Purishavaha Srotovikaras, which are disorders of the urinary and fecal systems. It emphasizes the role of Apana Vata and identifies common causes and symptoms of Srotodushti (channel vitiation). Treatment involves Shodhana (cleansing) therapies like Basti (medicated enema)—including Niruha, Anuvasana, and Uttara Basti—which are central for Vata-related issues like Mutrakruchra and Mutraghata. Snehana (oleation), Swedana (sudation), Virechana (purgation), and Vamana (emesis) are used depending on the Dosha predominance. Herbal medicines like Gokshura, Punarnava, Pashanabheda, Varuna, and Dashamoola are employed for their Mutrala, Ashmaribhedaka, and anti-inflammatory properties. Conditions such as Ashmari, Atisara, Pravahika, Purishaja Krimi, and Arshas are managed using a combination of Samshodhana, Samshamana (palliative), dietary regulation (Pathya-Apathya), and Rasayana therapies to restore function, reduce recurrence, and prevent complications.

This presentation on Manovaha Srotas explains the Ayurvedic concept of subtle channels governing mental functions, rooted in the Hridaya and influenced by Rajas and Tamas along with Sharirika Doshas. It highlights causative factors like Prajnaparadha, Asatmendriyartha Samyoga, and Parinama, which lead to various Manasika Vikara such as Unmada, Apasmara, Chittodvega, Vishada, and Atattvabhinivesha. Management follows a threefold approach—Daivavyapashraya (spiritual measures), Yuktivyapashraya (rational therapies including Shodhana and Shamana), and Satvavajaya (psychotherapy)—supported by Medhya Rasayana, Ahara-Vihara, and lifestyle regulation. A 62-year-old man presented with painful, burning vesicular eruptions on the right cheek, eye, and nose following an insect bite, diagnosed initially as herpes zoster. Despite allopathic and antiviral treatments, symptoms worsened, with pus discharge and sinus formation indicating secondary bacterial infection and Agni Visarpa.

Ayurvedic diagnosis confirmed Vata-Pitta predominant Agni Visarpa involving Rakta, Mamsa, and Lasika dhatus on Complete analysis of the presentation of disease it can be diagnosed as Kita Visha Damshtra[Early Cellulitis]. 

Treatment included Dashanga and Gairika lepa, Mahatiktaka ghrita, virechana with Trivrit Avaleha, and local wound management with Nimba kalka, leading to notable clinical improvement.

The case highlights the effectiveness of integrated Ayurvedic management including Shodhana and Shamana therapies in complex inflammatory skin disorders, especially when modern treatments fail. Early diagnosis, differential evaluation, and classical interventions proved crucial in preventing complications and promoting healing.

TREATMENT PROTOCOL OF ASTHIVAHA AND MAJJAVAHA SROTO VIKARA

This presentation elaborates on Asthivaha and Majjavaha Srotas—the margas responsible for the poshana of Asthi dhatu and Majja dhatu—along with their utpatti, karma, and srotodushti lakshanas. It describes samprapti and nidana of Asthigata and Majjagata rogas such as Asthisoushirya, Sandhigata Vata, Asthi-Majja Vidradhi, Bhagna, Khalitya, Palitya, Indralupta, Murcha, and Brama. Chikitsa is based on Vatahara and Brimhana principles, with prime importance to Tikta-Ksheera Basti, Snehana, Swedana, Upanaha, and Rasayana prayoga.

Management protocols include Basti karma (Sneha, Ksheera, Yapana, Tikta-Ksheera-Sarpi Basti), Nasya karma, Raktamokshana, and Agnikarma according to the dosha and dhatu avastha. Use of Guggulu kalpas, Sudha varga dravyas, Madhura and Tikta rasa aushadhis, and Balya-Rasayana yogas like Ashwagandha, Guduchi, Laksha, and Shilajatu is highlighted. Ahara vihara with pathya-apathya, vyayama and yoga asanas, and pranayama are advised to sustain dhatu samya and prevent punarbhava.

The presentation outlines Ayurvedic concepts and treatment protocols for disorders of Māṁsavaha Srotas (muscle channels) and Medovaha Srotas (fat channels), linking them to modern physiology such as the musculoskeletal system and adipose tissue metabolism. It details causes, pathogenesis, and symptoms for conditions like Galaganda, Gandamala, Apachi, Arbuda, Mamsa Sosha, Sthoulya (obesity), Karshya (emaciation), Urustambha, and Prameha (metabolic disorders including diabetes). Management involves removing causative factors, stimulating digestion, eliminating toxins, cleansing and nourishing tissues, and using Rasayana therapy for rejuvenation. Specific herbal medicines, diets, lifestyle modifications, and therapies like Udvartana, Snehana, and Panchakarma (with cautions in acute phases) are prescribed. The approach integrates traditional Ayurvedic principles with parallels to modern conditions such as metabolic syndrome, obesity, and muscle wasting, emphasizing holistic, sustainable restoration of tissue and channel health.

The case involves a 50-year-old male farmer diagnosed with Kaphaja Krimi Roga (intestinal worm infestation), presenting with whitish worms in stool, postprandial urgency, giddiness, and weight loss. He had multiple prior treatments, including allopathic and Ayurvedic interventions, with only temporary relief before recurrence of symptoms.Patient was diagnosed as Abhyantara Kaphaja Krimi [Trichinosis-Trichinella spiralis

Management included Deepana, Pachana, and Virechana karma , followed by internal medicines like Krimikutara Rasa, Vidangarishta, and Hingwashtaka Churna. These aimed to improve digestion, expel parasites, and restore gut immunity. The Virechana procedure removed morbid doshas, creating an unfavorable environment for parasite growth

The outcome showed significant improvement, with resolution of symptoms after therapy. The case underlined the importance of Ahara-Nidana Parivarjana (diet and cause avoidance), lifestyle regulation, and maintaining long-term digestive health to prevent recurrence. It also highlighted the scope of Ayurveda in managing recurrent parasitic infections and its potential link to gut microbiome modulation.
